Monocular Diplopia Associated with a Midbrain Stroke

چکیده
Background and Purpose Monocular diplopia is often due to intraocular disease. It is rarely the result of cerebral causes, which include lesions in or near Brodmann’s area nineteen, the frontal eye fields or the cerebellum. Monocular diplopia has not been reported in a patient with a midbrain lesion.

Could visual neglect induce amblyopia?
Oculomotor nerve disease is a common cause of diplopia. When strabismus is present, absence of diplopia has to induce the research of either uncovering of visual fields or monocular suppression, amblyopia or blindness. We describe the case of a 41-year-old woman presenting with right oculomotor paresis and left object-centred visual neglect due to a right fronto-parietal haemorrhage expanding t...
متن کاملIsolated Medial Rectus Nuclear Palsy as a Rare Presentation of Midbrain Infarction
BACKGROUND Diplopia is a common subjective complaint that can be the first manifestation of a serious pathology. Here, we report a rare case of midbrain infarction involving the lateral subnucleus of the oculomotor nuclear complex presenting as diplopia, with no other stroke manifestations. CASE REPORT An 83-year-old right-handed white man with past medical history of diabetes mellitus, hyper...
متن کاملMonocular Diplopia Caused by Ocular Aberrations and Hyperopic Defocus
As a single aperture, approximately monofocal optical system, the human eye generally creates a single image on the retina. However, the literature contains many reports of perceptual monocular diplopia. While it is easy to understand how distortion may produce monocular diplopia, its reported high incidence in normal eyes is less easily understood. We examine a model which ascribes monocular d...
متن کاملTeaching NeuroImage: Convergence spasm associated with midbrain compression by cerebral aneurysm.
A 72-year-old woman presented with intermittent diplopia. She had a history of vascular disease, but no history of stroke or psychiatric disturbance. On examination, she developed convergence spasm with associated miosis following fixation on near targets and during horizontal smooth pursuit (video, figure 1). The spasms were terminated with a blink. She had no ptosis, her eye movements were no...
متن کاملMonocular diplopia‐associated migraine‐like headache induced by nicotine withdrawal
We describe an extremely rare case of migraine-associated monocular diplopia developed in a 23-year-old man after sudden cessation of smoking. The physical examination and brain MRI scan were unremarkable. The symptoms resolved after starting nicotine patch. We reviewed the literature and discussed the diagnosis and possible mechanism of this phenomenon.
